Coventry City Council has been fined £125,000 after an 11-year-old girl was run over and killed by a reversing bin lorry. The court heard that just four months before the incident, a 15-year-old had escaped serious injury in similar circumstances, but the council failed to review its risk assessments as a result. In light of this, do you think the fine goes far enough? Contribute to the debate in our Discussion Forums.

A recent post from a user has sparked an interesting debate on exploring the boundaries between training and using your initiative in emergency. There is clearly no right or wrong answer, but what do you think?
